Dupont Hospital Corporate Wellness Program
The plan
The Dupont Hospital Corporate Wellness Program is an online lifestyle management program that helps
participants reduce their risk for heart attack or stroke and improve their overall health, fitness level and well-being by achieving and maintaining a proper weight, practicing good nutrition and exercising regularly and properly. Participants are introduced to scientifically-based behavior modification techniques in the areas of:
- nutrition
- exercise
- stress management
- prevention
- smoking cessation
The program offers unlimited flexibility since it is Web based and participants can access it at any time and from anywhere.
How does it work?
After your company enrolls in the program, each of your employees, their spouses and their dependants over the age of 18 will have access to the program. Each participant will have the opportunity to complete an online health history questionnaire (Health-Risk Assessment), have lab work done and body measurements taken. Once the tests are complete, an online letter notifies the participant of any risk factors in the areas of nutrition, exercise, stress management, weight management and smoking cessation. If the participant is at risk, he/she can opt to complete any or all of the five online self-help programs, one at a time, over the course of the year. High-risk individuals may choose to participate in the in-depth, one-on-one mentoring program that is offered separately.
Who is eligible to participate?
Your employees, their spouses and dependants age 18 and older are all eligible.
Participation and confidentiality
Once your company enrolls in the program, participation by employees and family members is voluntary and individual results are confidential. Outcome results are provided to the company in an aggregate form as well as individually to the participant.'
Your investment
As the employer, your investment in the program would entitle your participants access for one year to:
- the online Health Risk Assessment
- recommendations based on their HRA for goals and an action plan
- the five online self-help programs (weight management, nutrition, exercise, stress management, smoking cessation/smokeless tobacco)
- healthy recipes
Your investment also includes (for one year):
- a report with aggregate results
- a communication start-up kit
CD with templates for a letter to employees, a letter to senior leadership, flyer, table tent, poster, “Getting Started” instruction sheet for employees, and a personal log sheet for employees - a short video that shows your employees how to read their HRA results (coming soon)
For an additional cost, you may choose to either cover the costs of the labs (lipid profile, fasting glucose) and screenings (blood pressure, height, weight, hip, waist measurements) for your participants or pass that cost on to the individuals.
